In Mizuki's illustrations, she takes a horse-like form, with one eye, two horns, whiskers, and a belt of skulls.

Hōsō-Babaa is an ally of Kitarō, helping him scare the two men on the Ghost Train.

Hōsō-Babaa is a yōkai mentioned in the book Oshu Hanashi by Tadano Makatsu. She caused a plague of smallpox and then ate the corpses of people who died of smallpox.
In the early bunka era, there was a large outbreak of smallpox in the city of Oshu, at the same time, grave robbing and vandalizing graves was common, and thus this legend appeared.
